The following checklist is to be used by ESD Association Certified, ANSI/ESD S20.20 auditors for the purpose of performing Facility Certification Audits. The checklist reflects the ESD Program items that are required by the S20.20 document as well as addit ional certification requirements. The “D” column in the table refers to documentation requirements while the “V” requires that the assessor verify that the requirement is in existence and is working.

The following items are currently not included as program requirements in ANSI/ESD S20.20. However, each o f the items are generally considered by industry to be required, "good" practices. The following questions are additional, mandatory requirements of the Facility Certification Process.

The results of the audit can be broken down into three categories:

1. Observations - areas of concern that could lead to non-conformances (for Organization consideration only)

2. Minor non-conformance s - An isolated lapse of either discipline or control of one or more of the required system elements which does not

represent a breakdown of the ESD Control Program. (Minor non-conformances must be addressed by the Organization and responded to via

a Corrective Action Response to the Registrar before a Facility Certification ca n be issued.)

3. Major non-conformances - The absence of, or ineffective implementation of one or more of the required system elements. Any finding in this

category automatically results in non-certification of the Organization. A major non-conformance will require that the Registrar make a subsequent visit to the facility for a complete assessment.

主要不符合：一个或多个系统元素的缺失，无效。任何发现自动导致不符合。主要不符合项之后要求注册机构再次对工厂进行次完全审查。

The following table may be used to assist in the evaluation of the ESD Program technical elements used by the Organization.

The Organization should be able to demonstrate that actual measurements for each of the ESD Program Technical Elements are within the limits established in the Program Plan.
