Since the early nineties, many studies have been performed to examine the incidence of adverse events, with reported incidence rates ranging from 3 to 17% of all hospital admissions. A quarter to half of the adverse events was considered preventable. These studies have increased the sense of urgency to take effective countermeasures in order to improve patient safety in hospitals. Improvements can only be achieved if the interventions tackle the dominant under lying causes.
